A 17-month-old child died after becoming entrapped in the openings of a plastic indoor/outdoor slide playset — and the U.S. Consumer Product Safety Commission (CPSC) has issued an urgent warning to stop using the product immediately. CPSC is warning consumers to stop using Model WPX-4 Slide Playsets manufactured by Wenzhou Yidian Amusement Equipment Co., Ltd. because the playsets contain multiple openings where a child’s torso can fit through but a child’s head cannot — creating a fatal entrapment and strangulation hazard.
How the Entrapment Hazard Works
The WPX-4 playset is designed for both indoor and outdoor use and includes a tunnel, a slide, a basketball hoop, and in some versions a vehicle-shaped component on the side. The openings throughout the playset structure — including in the tunnel and frame sections — are sized in a way that allows a young child’s body to pass through while their head becomes lodged, causing neck entrapment. This is the same mechanism responsible for the documented death CPSC has received.
The playsets are available in red, navy, coffee, gray, green, and yellow colors.
What You Should Do
- Stop using Model WPX-4 Slide Playsets manufactured by Wenzhou Yidian Amusement Equipment Co., Ltd. immediately.
- Dispose of the playset — do not sell or give it away.
- When purchasing any children’s indoor or outdoor playset, verify that the product is certified to meet ASTM F963, the mandatory federal toy safety standard, and that it comes from a verifiable seller with a physical presence in the United States.
- Be especially cautious when purchasing children’s play structures from unknown sellers on Amazon, TikTok Shop, Shein, or other online marketplaces — these platforms have been repeatedly flagged by CPSC for selling products from Chinese manufacturers that violate U.S. safety standards.
- When evaluating any climbing or play structure, check that all openings are either small enough to prevent a child’s head from entering or large enough for the child to pass through completely — never in between.
